Clark This Toolkit is the companion text to the Service Access Models: A Way Forward Resource Guide for Community
Health Services. The Toolkit consists of sample documents, policies, templates and checklists to assist Community
Health Service (CHS) managers with the review, selection and implementation of Service Access models and
systems. Sample workplans and change management tools are also included. It is anticipated use of these
resources will assist CHSs to progress the embedding of integrated Service Access models and further streamline
consumer access to services.
The Toolkit consists of 6 sections, incorporating:
• Service Access model implementation support tools
• Review of current Service Access infrastructure, systems and practice
• Model conceptualisation and consumer pathways
• Implementation (and evaluation)
• Policies, procedures and workforce support
• Templates & Checklists
